  • Claim-Specific Information (description of injury, claim #, authorization #, physician info, etc.)

  • Treatment Location

  • Therapy or Service Visit Details

  • Insurance-Specific Information

  • Other Relevant Information

Note: In accordance with HIPAA, we will not disclose to third parties personally identifiable or medical information regarding patients uploaded by our clients or providers to the Make a Referral portion of the Web Site, except (i) to those of our providers that may contact or interact with such patients, (ii) to the client that introduced such patients to our services, or (iii) when we determine in good faith that it is required by law.

Although we believe we would be better able to service your needs by collecting your information, you may choose to visit our site anonymously.
